
General Terms and Conditions - Consumer Rebates
All rebate programs are valid only for the rebate period stated on the rebate form. Dealers are not authorized to extend the rebate period. Rebate requests must be postmarked no later than seven days after the rebate period expires unless otherwise stated on the rebate form. To receive your rebate, you must complete and mail/fax: (a) Original rebate form, (b) Original warranty card, (c) Copy of the sales receipt. This offer is valid only on Unitized Retail Package purchases of the products identified on the rebate form. The rebate applies only to those model numbers specifically listed on the rebate form. No rebate can be paid without a valid serial number. The serial number can be found on the bottom of the unit or on the exterior of the carton. Only one rebate is paid per serial number. If you receive a product that has been opened, we strongly recommend that you or the dealer confirm that no rebate has been paid on this unit. If a rebate has already been paid on a product, Hayward will not pay another rebate. It will be the dealers's responsibility to pay any rebate that has been promised.
Please keep a copy of your receipt. All requests (and all documentation submitted with them) will become the property of Hayward. Documentation will not be returned to the customer. Your request for a rebate must be made on an original rebate form. Rebates forms cannot be altered in any way. The rebate form is not transferable. It is only valid for the person named on the form. Copies of this form will not be accepted. Dealers are not authorized to copy this form or to put it on a web site. If this rebate form is printed from a website other than the official www.haywardrebates.com web site, it will not be accepted. On-Line Rebate forms generated by consumers are numbered and may only be used once . Duplicate requests will not be honored. Rebate checks will not be mailed to a Business address. The physical location of the pool and the name of the product owner must be provided to receive a rebate. This offer is available to retail customers only. Dealers, builders, service professionals, and distributors are not eligible for this promotion.
Theft, diversion, reproduction, sale or purchase of this form is prohibited. This offer is good only in the USA and only if the product is purchased from a Hayward Authorized Hayward Dealer. Offer is void where taxed, restricted or prohibited. Limit one request per household. Hayward is not responsible for mail that is late, lost or not sent to the address listed above. Please allow 6 to 8 weeks for processing.

All rebate inquiries must be made within 1 year of the purchase date. No rebate payment will be made on purchases made more than 1 year ago. Hayward reserves the right to change or cancel this offer without prior notice.
